Dear Families

As you may know the NHS is now offering Covid-19 vaccinations for 12-17 year old students.

We have been told today that our date will be this Friday, 8th October 2021. The vaccinations will be delivered by the NHS school vaccination team at the Academy in the same way we have previously offered HPV vaccinations.

I attach a letter from the NHS which contains an e-consent form for you to complete if you wish your child to be vaccinated. It also contains information from the government and NHS about the programme for both yourself and your child.

This consent is directly with the NHS and no details are held by the Academy. Students in Year 7 who have not yet turned 12 will not be eligible at this time. If you are trying to complete the form but struggling ICT wise, if you call into reception we will try and assist.

I apologise for the short notice, I was only contacted by the service this afternoon. If you do not wish your child to be vaccinated please simply do not complete the form and the NHS staff will not include you on their lists.
